• Authorization for Administration of Medication: Blair Academy Health Center requires a physician’s written order to administer medications.
• Authorization of Medical Treatment: For the safety of your child and the protection of the School, no student will be allowed to register until this form is completed and on file.
• Group Immunization Patient Informed Consent Form: Consent form to administer FLUOGEN (Influenza Virus Vaccine, Trivalent, Types A and B).
• Hepatitis B Vaccine: Information and consent form to administer the Hepatitis B vaccine.
• Medical Form A – Physician’s Report: Must be completed for each student prior to each year at Blair.
• Medical Form B – Medical History: To be completed by new students and parents and reviewed by your physician.
