Please complete the form below to request a copy of your Blair Academy transcript. Please note in submitting this form, you are authorizing Blair Academy to provide your transcript to the persons/agencies you list. We ask that you allow three to five days for us to process your transcript.

Current students: If your summer school application or transfer request requires a copy of your transcript, please submit this online transcript request form, including program details in the additional information section below. If you need a transcript for colleges or the NCAA Eligibility Center, please submit this online transcript request form prior to contacting College Counseling.
