How to Access August Schools
Follow these steps to access August Schools
Step 1
Below is an orange button labeled “Complete Blair's 2024-2025 AUGUST SCHOOL FORMS.” Click this while holding “control” and select “open link in new tab” and you will be granted secure access to the links and forms needed to complete Blair’s enrollment process (while not losing the instructions page you started on and will need to refer to throughout the process). More detailed information regarding health center policies can be found in the Student Handbook.
Step 2
You will now be logged into the August Schools system so you can access and print all required forms (New Jersey state pre-participation physical form, tuberculosis screening form and immunization form) and any other forms that may apply to your child (see below). Bring these printed forms to your healthcare provider. The healthcare provider must sign, date and provide an office stamp where indicated. The first time you log into the August Schools system, you will be taken to an information screen. If this is your first time logging into August Schools, please click here for instructions. Once the forms are completed, go back into the Parent Portal, select the August Schools Forms section once again, and upload the documents so they will be filed directly into August Schools. If a document has multiple pages (such as the physical file), please upload all pages together as one file.
Step 3
Establish an account with Acme Pharmacy. This is important, even if your child does not take daily medications. In the event your child becomes ill or needs over-the-counter medications or prescriptions during the academic year, the Health Center will obtain both over-the-counter and prescription medications for your child from Acme Pharmacy, which is located a few miles from campus.

The pharmacy will not dispense any medication without prior payment.
If your child takes multiple medications daily we encourage you to find a pharmacy who will package the medications for daily use. All controlled medications must be blister packed.
- It is very important to keep your payment information current, as the pharmacy will not be able to dispense any medications/supplies without payment. We do not want your child to have to wait for medication until someone at the pharmacy reaches you. All prescriptions for controlled substances must be blister packed; no exceptions.
- In the event you choose to use a different pharmacy, please note the following: All prescriptions must be purchased from a United States pharmacy and remain in the original labeled container from that pharmacy. International students taking prescription medication may require assistance with this policy and should contact the health center for prescription support at healthcenter@blair.edu.
- If you choose to use a different pharmacy, all controlled substances must be blister packed by the pharmacy; there will be NO EXCEPTIONS.
- A Medication Order Form must be completed by a licensed prescriber for each medication and with any subsequent order changes. This form can be obtained from the health center and the Magnus website. This form is legally required for all prescription medications your child may be taking before they can be dispensed.

Required Forms/Requirements for All Students to Bring to your Doctor’s Office (to be completed & signed)
- Pre-Participation Physical Evaluation Form Physical Exam Form*
- The physical must be current and must occur between June 1 and August 1, 2024. When you schedule your child’s sports physical, please verify that the healthcare provider who will perform the exam has completed the Student-Athlete Cardiac Assessment professional development module (PD module). If the healthcare provider has not completed this module, we will not be able to accept your child’s physical. As of June 16, this is a New Jersey state requirement. NJ S2518 is currently pending Assembly Education Committee review. Once it passes, we will modify the requirements to be in compliance with New Jersey state law.
- Tuberculosis Screening*
- Blair Academy requires that all incoming students be screened for tuberculosis. Please ask your doctor to complete this screening at the same time as the physical.
- Immunization Form*
- All students must meet the New Jersey immunization requirements for Tdap, MMR, hepatitis B, polio, varicella, DTaP and meningococcal ACYW as outlined on the Blair immunization form. Please note we will be holding a flu clinic this fall provided by Acme Pharmacy. Parents will receive an email with more information and electronic consent form to be completed later in the school year.
- *Students with medical exemptions for immunization must provide an annual note updated by their medical provider.
- *Students with religious exemptions must provide an annual letter stating the religious exemption.
- *Immunizations must be up-to-date and meet New Jersey state requirements. If a required vaccine is unavailable at your place of residence, you will need to establish a relationship with a provider in the United States and obtain the vaccine PRIOR to your arrival at Blair. No students will be allowed on campus or nor will they be permitted to move in without proper documentation of required immunizations.

Additional Forms to Print to Bring to your Doctor’s Office (to be completed & signed)
ONLY IF any of the following forms apply to your child, please print and bring them with you to his or her physical for your healthcare provider to complete and sign. (These forms would be necessary if, for example, your child is taking prescription medications or controlled substances or if he or she requires an action plan for asthma, allergy, diabetes or seizures.)
- Medication Order Form, if your child takes prescription medications
- Readmission Form, if your child has had a recent extended illness, orthopedic surgery or stitches
- Asthma Action Plan Form (If required, one inhaler is to be stored in the health center and one for your child)
- Food Allergy Action Plan Form (which is required even if your child does not use an epinephrine pen). In addition, if your child has an allergy but does not require medication, please indicate “no medication required” and still submit the form. (If an epi pen is required, one will be stored in the health center and one carried by your child)
- Diabetes Action Plan Form
- Seizure Action Plan Form
Counseling Services (if applicable)
Students receiving mental health services at home should have ongoing care during the academic year. If you need assistance coordinating mental health support while at Blair, we encourage you to contact Blair’s Director of Counseling Ally Thomas at (908) 362-6121, ext. 5687, or thomaa@blair.edu, prior to registration.
